ABSTRACT Aim: To investigate the histopathologic changes in native bladder and gastrointestinal segment, the relation between histopathologic changes, type of operation and the period passed over operation in patients with bladder augmentation. Materials and methods: Twenty consecutive patients were enrolled in this study. Histopathologic evaluation of the cystoscopic mucosal biopsies from native bladder and enteric augment was performed in all patients. Results: Active or chronic non-specific inflammation of various degrees was found in all specimens except two. Metaplastic changes were detected in 3 patients. Two patients had squamous metaplasia (one focal, one extensive) and one patient had intestinal metaplasia. All metaplastic changes were found in native bladder specimens. The type of augmentation in patients with metaplastic changes were ileocystoplasty and sigmoidocystoplasty. No signs of malignancy were detected in any patient. Conclusion: The complexity of the disorders requiring bladder augmentation does not let the surgeons to draw a clear line between different groups of complications including malignancy formation. However, due to challenging course of the augmentation procedure itself, surgeons should be well aware of the possibility of malignancy development.

PURPOSE: To develop an experimental model of infravesical urinary obstruction in female rats. METHODS: After median caudal laparotomy, the urethra of 14 female rats was delicately separated from the vagina and loosely wrapped with cellophane tape measuring 0.4 x 1.0 cm. The animals were evaluated 4 (n=7) and 8 (n=7) weeks later. Five additional control animals were only subjected to separation of the urethra and vagina and monitored for 12 weeks. RESULTS: After four weeks, three rats presented vesical dilation associated with discrete ureteral ectasis in 2 animals, with the third presenting discrete hydronephrosis in one kidney. After eight weeks, five rats (71.4 percent) presented vesical distension with bilateral ureterohydronephrosis. No significant changes (p>0.05) in serum urea or creatinine occurred in any group compared to preoperative values. CONCLUSION: We present here an inexpensive and simple method for the slow induction of urethral obstruction in adult female rats, with the development of progressive vesical hypertrophy and ureterohydronephrosis, which may be used as an experimental model for the study of different aspects of obstructive nephropathy.


OBJETIVO: Desenvolver modelo experimental de obstrução urinária infravesical em ratas. MÉTODOS: Após laparotomia caudal mediana, as uretras de 14 ratas foram delicadamente separadas da vagina e frouxamente envoltas com fita de celofane medindo 0.4 x 1.0 cm. Os animais foram avaliados 4 (n=7) e 8 (n=7) semanas depois. Cinco animais adicionais (controle) foram submetidos apenas à separação da uretra e da vagina e monitoradas por 12 semanas. RESULTADOS: Após quatro semanas, três ratas apresentaram dilatação vesical associada a discreta ectasia ureteral em 2 animais, com o terceiro apresentando discreta hidronefrose em um rim. Após oito semanas, cinco ratas (71.4 por cento) apresentaram distensão vesical com ureterohidronefrose bilateral. Não ocorreram alterações significativas (p>0.05) nos valores de uréia ou creatinina em qualquer grupo, comparado com valores pré-operatórios. CONCLUSÃO: Apresentamos aqui um método barato e simples para a indução lenta de obstrução uretral em ratas adultas, com desenvolvimento progressivo de hipertrofia vesical e ureterohidronefrose, podendo ser utilizado como modelo experimental para estudo de diferentes aspectos da nefropatia obstrutiva.

RESUMO

A síndrome megabexiga microcólon hipoperistaltismo intestinal (SMMHI) é uma doença rara, de provável herança genética, com prognóstico reservado, já no primeiro ano de vida. Relata-se um caso de SMMHI em criança do sexo feminino, atualmente com 16 anos de idade, com o objetivo de ressaltar o prognóstico favorável, não esperado, com sobrevida até a adolescência, de uma síndrome rara. Chama-se a atenção para a possibilidade de estabelecer o diagnóstico no período perinatal e para a importância de realizar o estudo urodinâmico.

RESUMO

Neonatal bladder rupture is rare as a complication of bladder obstruction due to abnormal anatomy or iatrogenic cause such as umbilical catheterization. The present study describes the case of a 27-day old infant with ascites due to bladder perforation secondary to bladder wall necrosis as a result of severe urinary tract infection. The baby was treated aggressively with antibiotics and underwent successful surgical repair of the perforation.

RESUMO

Paciente de 36 anos de idade, proveniente de zona endêmica foi admitido com episódios de hematúria macroscópica total e disúria terminal. A citoscopia mostrou um tumor exofítico na parede anterior da bexiga, sugestivo de câncer. A biópsia revelou esquistossomose mansônica. Ressecção transuretral confirmou o diagnóstico. O paciente foi clinicamente tratado com oxamniquine, obtendo cura. O caso apresentado nesse trabalho enfatiza que nem todos os tumores de bexiga com hematúria microscópica são cânceres de bexiga.

In order to study the pathogenesis, clinical and pathological characteristics of proliferative lesions of the bladder, 50 cases of proliferative lesions of the bladder from 150 patients with complaints of frequency, urgency, hematuria and dysuria were subjected to cystoscopic biopsy of the suspicious foci in the bladder. In combination with the symptoms, urine and urodynamics, the relationship of proliferative lesions of the bladder to the inflammation and obstruction of the lower urinary tract was analyzed. Of the 50 cases of proliferative bladder lesions, 44 cases (88%) had lower urinary tract infection and 29 (58%) lower urinary tract obstruction. The patients with lower urinary tract obstruction were all complicated with infection. Three cases were associated with transitional cell carcinoma. Malignant cells were detected in 1 case by urinary cytologic examination. Proliferative lesions of the bladder, especially those without other obvious mucosa changes under cystoscopy, are common histological variants of urothelium in the patients with chronic inflammation and obstruction of the lower urinary tract. Chronic inflammation and obstruction of the lower urinary tract might be the causes for proliferative lesions of the bladder. It is suggested that different treatments should be applied according to the scope and histological type of the proliferative lesions.

This case reports muellerianosis of the urinary bladder, showing glandular lesions made up of endocervical type glands, in a 36-yr-old woman. The patient presented with lower abdominal discomfort and pain on voiding. The patient had undergone two Cesarean sections 5 yr and 3 yr earlier. On a pelvic ultrasonography, a well-circumscribed mass, 2.2x0.8 cm in dimension, was found with luminal polypoid projection in the posterior wall of the urinary bladder. The patient had a transurethral resection of the bladder mass. Histologically, the tumor was composed of irregularly shaped glands lined by endocervical mucous epithelium in the muscularis propria of the urinary bladder. Some glands exhibited cystic dilatation and contained mucinous secretions. The glands elicited no desmoplastic tissue reaction. The intraluminal mucin often contained polymorphonuclear leukocytes. The glands were mostly lined by tall columnar and bland looking mucous cells with mucin secretion. The ciliated cells were rarely observed. No endometrial component is noted. It would be appropriate to designate this lesion as "Muellerianosis of the urinary bladder, endocervicosis type". Awareness of the lesion and attention to its typical histologic features should facilitate its crucial distinction from adenocarcinoma.

RESUMO

A previously healthy 44-year-old male was admitted with the chief complaint of intermittent gross hematuria. On initial ultrasonographic and CT examination, a grossly protruding intravesical tumor was noted and, under the impression of a malignant bladder tumor, transurethral resection was performed. The histological findings were spindle cells with elongated cytoplasm with rare mitotic figures distributed in myxoid stroma, consistent with diagnosis of inflammatory pseudotumor of the bladder. The benign nature of this tumor warrants conservative surgical management, usually consisting of transurethral resection or partial cystectomy. No reports of metastasis have been reported following complete excision. Therefore, any suspicion and recognition of this entity is imperative to avoid performing an irreversible radical procedure.

RESUMO

The inflammatory pseudotumor of the urinary bladder is rare, especially in children. It is a benign proliferative lesion of the submucosal stroma easily mistaken for a sarcoma clinically, so it should be differentiated from a malignant neoplasm. We report the case of bladder inflammatory pseudotumor in a 7-year-old girl.

Os mega-orgaos na Doenca de Chagas sao bem conhecidos, especialmente os desenvolvidos no sistema digestivo. A infeccao aguda apresenta parasitismo de diversas celulas, tecidos e orgaos, dentre eles a bexiga urinaria. Camundongos Balb/c infectados com 100.000 formas sanguineas de cepa Y de T. cruzi mostraram intenso parasitismo de todas camadas da bexiga urinaria na fase aguda. Os parasitas foram encontrados na mucosa, submucosa, lamina propria, muscular, adventicia e tecido adiposo, alem das celulas descamadas para a luz do orgao...

To investigate the histopathological characteristics of inverted papillomas of the urinary bladder, including the possibility of malignant transformation, we studied the indicators of cellular proliferation activity in 7 inverted papillomas of the bladder including two cases of malignant inverted papilloma of the bladder. PCNA expression rates in two cases of malignant inverted papilloma were higher than in benign inverted papillomas. Mean numbers of AgNORs per nucleus in malignant inverted papillomas were much more than in benign inverted papillomas. The c-erbB-2 oncoprotein was expressed only in malignant inverted papillomas. These results suggest that PCNA expression rate, mean number of AgNORs per nucleus and c-erB-2 oncoprotein expression may be merited as good indicators to detect the inverted papilloma with more proliferative and aggressive lesions, and with the potential of malignant transformation.

RESUMO

For cases of parasitic diseases have been diagnosed by preparing samples with the plasma-thrombin cell block techinique. This method has been previously applied for the cytological detection of cancer. The authors want to emphasize that by applyng this method to samples obtained from parasitized organs, it is posible to diagnose easily the disease as well as to study, by means of pseudotissural preparations, the tissues response to the parasite and, with follow up samples, the progression of the disease itself.
